Job Description:
A company is looking for an RN Care Coordinator to support
patients in various clinics through a hybrid work schedule.
Key Responsibilities
Coordinates care efforts for effective patient transitions and
provides pro-active disease management
Acts as a liaison between patients, families, and medical staff to
promote patient-centered care
Establishes nursing care plans, provides education, and evaluates
patient progress
Required Qualifications
Licensed RN in the State of MN
Three years of specialty experience in care coordination or five
years of clinical experience
Basic Life Support certification within 30 days of hire
Preferred four-year Baccalaureate degree in nursing
Preferred two years of RN experience in the specialty area or
previous care coordination experience
